GENERAL INFORMATION
Personal data owners defined as “data subjects” under Personal Data Protection Law No. 6698 (“PDPL”) may submit requests concerning their rights under Article 11 to the data controller. Under Article 13 of the PDPL, applications must be submitted to the data controller in Turkish. This English translation is for information and convenience only; in case of discrepancy, the Turkish version prevails. For this form, BANDO CREATIVE STUDIO REKLAM A.Ş. (TIN: 1400578014) (“Ba'ndo”) is the data controller.
Application Methods

It is recommended that the mail/email subject line state "PDPL Data Subject Application" and that the application clearly identify the request made under Article 11 of the PDPL.
Applications submitted without a printout of this form must include the applicant's full name, signature (for written applications), Turkish ID number (for foreign nationals: nationality, passport number or ID number, if any), residential or workplace address for service, email address, telephone and fax numbers, and the subject matter of the request, as required by the Communiqué on the Procedures and Principles of Application to the Data Controller (“Communiqué”). Under Article 13(2) of the PDPL, your application will be answered, depending on the nature of the request, as soon as possible and no later than 30 (thirty) days after it reaches Ba'ndo. The response will be delivered to you in writing or electronically in accordance with Article 13 of the PDPL.
If processing your application entails an additional cost, you will be required to pay the fee specified in the Communiqué issued by the Personal Data Protection Board. For a written response, no fee will be charged for the first 10 (ten) pages; a processing fee of TRY 1 will be charged for each page thereafter. If the response is provided on a recording medium such as a CD or flash drive, you will be charged the cost of that medium.
We reserve the right to amend the application procedures in line with Board decisions or future legislation. If the Board announces another application method, information about that method will be published on Ba'ndo's website.
This form has been prepared to identify your relationship with Ba'ndo and, where applicable, the personal data processed by Ba'ndo so that your application can be answered accurately and within the statutory period. To prevent legal risks arising from unlawful or improper data disclosure and, in particular, to safeguard your personal data, additional documents and information (such as a copy of an identity card, passport or driving licence) may be requested to verify identity and authority.
If information supplied in connection with your requests is inaccurate or out of date, or if an unauthorized application is made, Ba'ndo accepts no liability for requests arising from such inaccurate information or unauthorized application.
